Instead of saying "sorry", you prefer to give an explanation! But no matter how comprehensive you are, it is aimed at defending your actions and exonerating yourself which the other party finds offensive most times.
That is, if you do not complicate issues by wrong expression or exaggerated emotions.
Saying sorry simply means you accept that the fault or failure is as a result of your irresponsibility though, at times some things are beyond our control.
But then, for the fact they are our responsibility we take responsibility of whatever their outcome maybe! It does not make us failures... Just say "sorry"!
